The highway's northern terminus is in O'Neill, Nebraska at an intersection with U.S. Highway 20 and U.S. Highway 281. Its southern terminus is near Rock Port, Missouri at an intersection with U.S. Highway 136. US 275 crosses into Iowa near Hamburg. After coming into Hamburg from the east, it turns north at an intersection with Iowa Highway 333. It goes north, meets Iowa Highway 2 and the pair overlap until separating east of Sidney, Iowa. It continues north through Tabor and meets U.S. Route 34. It turns west with US 34, passes by Glenwood, Iowa, then meets Interstate 29. It turns north with I-29, and the two routes overlap until the Iowa Highway 92 exit at Council Bluffs, Iowa. It then turns west with Iowa 92 and crosses the Missouri River over the South Omaha Veterans Memorial Bridge.
